HONORS CONTRACT
Guidelines and Forms for Honors Contracts Students can use this procedure to convert a regular course into an Honors course if graduation requirements prevent the student from registering for an Honors course.
QUESTIONNAIRE
Senior Profile Questionnaire This questionnaire will be used to collect graduating senior responses which will be included in a profile created for the current academic year. This form is to be completed during the last semester.

All thesis advisor forms, proposals, including funding requests, must be submitted via the Honors Program Canvas Course.
The Honors Thesis Proposal and Research Funding Request should be completed in consultation with the Honors Thesis advisor near the beginning of the research process.
Honors Thesis Advisor Selection Form Juniors must notify the Honors Program of their Honors thesis advisor by the end of the first semester of their junior year by completing this form.
Application for Funds in Support of Honors Thesis Research
Once funding is approved, thesis research supplies* should be ordered through University Laboratory Managers:

* For those departments without a Laboratory Manager, the Honors Program will assist with the ordering of supplies.
Honors Thesis Progress Report The Honors Thesis Progress Report should be submitted to the Honors Thesis advisor, who will then forward it, with comments, to the Honors Program Director.
Honors Thesis Title Page This is the template for the Honors Thesis title page.
Honors Thesis Library Permission and Access Form Students who wish their Honors Thesis to be posted in Digital Commons @ New Haven, the University of New Haven’s institutional repository, should submit this form.
