GSC Forms

The following forms can be used by recognized student organizations and individuals (seeking conference reimbursement only) to request GSC Funds.

Operating Funds Request Form–  for general operating funds of RSO’s (not specific to one event or project).  Graduate Student Clubs/Organizations who seek operating funds must meet the following criteria:

  • The RSO must fill out an Operating Funds Request Form (below).
  • Operating Funds Request Forms must be handed into the GSC Treasurer no later than the second general meeting of each trimester.
  • Money will not be allocated above and beyond the provisions in the GSC general budget.

Miscellaneous Request Form– to support a one time only activity or a specific program of the RSO. Graduate RSO’s who seek miscellaneous funds must meet the following criteria:

  • The RSO must fill out a Miscellaneous Request Form (below).
  • Money will not be allocated above and beyond the provisions in the GSC general budget.

Conference Reimbursement Form– to reimbuse individual students for costs associated with attending conferences directly related to their academic and professional development.   Individual Graduate students who seek conference reimbursement must meet the following criteria:

  • The student must fill out a Conference Reimbursement Form (below).
  • The student must provide proof of payment for the conference.
  • All forms must be submitted two weeks prior to the end of the fiscal year.
  • Money will not be allocated above and beyond the provisions in the GSC general budget.
  • The conference must be related to professional or academic development.
  • Reimbursement shall be for registration costs only for a maximum of $75.00.
  • Reimbursement shall only occur once per academic year.
