The above forms are hosted in Adobe Sign. When prompted for an email address, you must use your University of New Haven account (newhaven.edu). Once the Payroll Office approves your completed form, you will receive another AdobeSign confirmation within 2 business days. If you do not receive a confirmation email after two business days, please contact us at payroll@newhaven.edu. For each form you complete, a verification message will be sent to your inbox.
